Duties & Responsibilities: 

• Initial admission and assessment of client needs and implementation of ongoing care, co-ordination, case management and care planning.

• Drive to and from client visits in your own car (transport allowance provided)

• Delivering exceptional clinical nursing care to client following the practice standards and guidelines.

• Wound care and catheter management.

• Provide written and verbal reports on well-being of clients.

• Liaise with various medical teams to facilitate appropriate client-centred care.

• Current Medication Administration HLTHPS006

• Current Australian Driver's Licence and Car

• Comprehensive Vehicle Insurance

• Current National Police Check

• Current Working with Children's Check (Employee Type)

• Current NDIS Worker Screening Check
